Start with a clinical note

Paste documentation or choose an example. Matched phrases will appear in the note as you type.

Highlights show candidate discovery, not verified evidence. Scores are model outputs, not clinical probabilities. Only codes mapped to the selected HCC scope are evaluated. Medicare and ACA tags are separate mappings, not patient risk scores; an asterisk marks a conditional mapping.
Review your note, then analyzeThe local model checks the complete note for each candidate.

How should diagnoses be reviewed?

These choices affect candidate discovery and model questions, not the text of your note.

Check each complete diagnosis against the full note.

Propose combinations such as diabetes with neuropathy. These remain hypotheses for review. Changing this clears scores; the loaded model is kept.

A quick guide

Review the complete note alongside its diagnosis candidates.

  1. Add a note. Paste your documentation or choose a synthetic example.
  2. Choose your scope. Medicare V28, ACA HHS 2026, or Both determines which mapped codes are reviewed—not every possible diagnosis.
  3. Load, then analyze. Select Load local AI. Once it is ready, the same button becomes Analyze note. Select it to review the full note; loading alone does not start an analysis.
  4. Inspect the wording. Click a diagnosis or source phrase to focus its highlights. Highlights explain discovery; they are not proof of current disease.
  5. Select and copy. Review the suggestions and choose the codes you want to copy. Scores are model outputs, not clinical probabilities.

Edit the decision prompt

One question per diagnosis or required detail. Your full note and the no/yes options stay unchanged.

Keep {diagnosis} for the diagnosis or required detail. Optional: {code} and {icd_description}. A detail question may have no single code; its description is the detail text. Complete-diagnosis questions keep the original ICD description. Edits apply to every question in this tab.

Stage, laterality, subtype, and complications are never removed by this option. The model still makes a no/yes decision; it does not generate a rationale.
