About NoteCoder
A second look at your documentation.
NoteCoder brings diagnosis candidates, relevant wording and code descriptions into one compact workspace. A local model helps you review the complete note; you decide what is actually supported.
Find and connect
Phrase matching finds potential diagnoses. Stage, side, site and compound-condition details may suggest additional candidates. Highlights show where a proposal came from, not proof that it is correct.
Review locally
The model checks candidates against the complete note in your browser. Note content is not sent to a remote model by this interface. First use downloads model files, which may be cached on your device.

The selector controls which mapped ICD-10-CM codes are offered, not which diagnoses a patient has.
- Medicare V28: codes associated with CMS-HCC V28.
- ACA HHS 2026: codes associated with the 2026 HHS-HCC model, a separate mapping used for ACA risk adjustment.
- Both: the union of these mappings. A code appears once, with the appropriate Medicare and ACA tags.
Tags identify mapping membership. They are not patient risk scores, proof of payment eligibility or a completed risk-adjustment calculation. ACA age/sex edit conditions require separate review.
Designed for a careful reader
Inspect the note beside the suggestions, examine the source phrases, adjust the display cutoff and copy selected codes with their descriptions. The built-in examples are synthetic, so you can explore without entering a real patient note.
Missing candidates and incorrect suggestions are possible. Always review the documentation and applicable code requirements before using a result.
Data and technical notes
The ICD-10-CM catalog is pinned to the FY2026 April 1 update; it is not a claim that every future code update is included. Medicare mappings use CMS-HCC V28. ACA mappings use the official 2026 HHS-HCC V08 source packaged with this release.
The ACA source supports January–September 2026 ICD-10-CM mapping coverage. Its FY2027 column is empty, so new October–December 2026 codes and mappings are not included in this release. Do not assume full-year coverage of newly introduced codes.
Verify the source releases at CMS: 2026 Medicare model software and ICD-10 mappings, and Marketplace regulations and guidance (July 31, 2026 benefit-year DIY software).
Local inference uses a browser WebAssembly runtime. Model startup, hardware support and speed depend on the browser and device; the app checks availability before loading. Balanced is an experimental reduced-compute model, while Full and Fast are alternatives. The names are not an accuracy ranking.
No hierarchy, demographic coefficient or patient-level risk score is calculated by the displayed mapping tags.
